1) It is distributed once a year at the end of Fall Term for students to fill out and submit.
2) The information indicated on this form is recorded by our office and serves as serious indication of the student's choices of elective courses. Change of selection choices after filling this form will require special appointments with the Program Director.
3) The form is for survey purposes but not for registration. Registration for elective courses will still be conducted via e-mail at the beginning of the Winter and Spring/Summer term.
